What happens after I make a referral?

We will get back to you: Once you submit your referral, we will get back to you within 2 business days
We will have an initial conversation with you to understand your goals, preferences and care needs aligned to the NDIS plan and Thrive360 Care capacity.
If this progresses then we will organise a planning meeting with you/and your supports to help design a Thrive360 Care support plan.
We will then match you with the right supports, including carers or clinicians and ensure they have the right training and resources for you.
Supports will then start!
